      Which health plans do you accept?

      We accept MOST private/commercial medical insurances. We do not, however, accept Magnacare, Amerigroup NJ, and Oxford Metro Plan by United health Care. We accept Medicare and all NJ State Medicaid programs except Amerigroup NJ.

      What do I need to bring to my first primary care visit?
      • During your first Primary Care visit, you will be asked to present a valid government issued identification document with your name appearing exactly as it does on your health insurance card. 
      • If your health insurance requires that you assign a primary care provider (e.g. all state Medicaid programs), it’s your responsibility to contact your health insurance and designate either Dr. Deval Gadhvi or Dr. Prahlad Gadhavi as your Primary Care Provider (PCP). 
      • If you have previous medical records, be sure to bring all relevant documents.
      Can I request a refill of my prescription?
      • New Patient – if you’re a new patient, you’ll need to be seen by one of the providers for an evaluation. Based on the evaluation you may either be approved for your regular prescription, a change in dosage or a medication that may be more appropriate for you.
      • Existing Patient – if you are an existing patient, uncontrolled substances will be filled for a 90 day duration. After which, a follow up will be necessary to evaluate need for adjustment of dosage or continuation of therapy
      • Controlled Substance – in the case of a controlled substance, the law allows a maximum of a 30 day prescription after which you will need to be seen by the provider for subsequent refills.

      In the event of unforeseen/unplanned circumstances, you may email the practice to request a refill

      What is a prior authorization (medication/procedure), and how does it work?

      Obtaining prior authorization is a complex process that involves our medical team working with doctors contracted by your insurance to determine your eligibility for certain diagnostic imaging, medications, procedures, or referrals. The determination is based on your insurance benefits/coverage, and the medical necessity based on guidelines provided by your insurance and documentation we submit.

      The following is a short list to demonstrate what typically needs a prior authorization:

      • Imaging: CT scan, MRI
      • Procedures: Biopsy
      • Medication: Any medication that is not in covered by your insurance coverage
